1 form - multiple email addresses based on radio?

    Is there a way to create a form that will send the results to a specific email address based on a radio button selected?

    For example, someone signing up in CA would select the radio button for CA and their submission will go to CA@Domain.com while someone for OR would select the OR radio button and their submission would go to OR@Domain.com?

    This would save me from having to make multiple pages for and links just for a form.

    Phil, unless someone here has a solution, you may want to look into using Wufoo forms (we have an interfacing widget) or Jot Forms, which is pretty flexible and popular.
